| Description | Defensins are cysteine-rich cationic polypeptides that are important in the immunologic response to invading microorganisms. The antimicrobial protein encoded by this gene is secreted and is a member of the beta defensin protein family. Beta defensin genes are found in several clusters throughout the genome, with this gene mapping to a cluster at 20p13. The encoded protein is highly similar to an epididymal-specific secretory protein (ESP13.2) from cynomolgus monkey. Mutation of this gene is associated with impaired sperm function. Its unique structure—comprising a conserved β‐defensin core and a heavily glycosylated, extended C‐terminal tail—is essential for multiple aspects of sperm function. DEFB126 facilitates the smooth transit of sperm through the viscous fluids of the female reproductive tract by promoting efficient cervical mucus penetration and by mediating binding to oviductal epithelial cells, thereby contributing to the formation of a sperm reservoir (1,2,4,16). Moreover, common sequence variants, including small deletions that lead to aberrant mRNA and truncated or unstable protein products, are associated with diminished sperm surface glycosylation, reduced ability to traverse mucus, premature capacitation, and ultimately lower fertilization rates in both clinical and experimental settings (1,5,7,22). In addition to its role in promoting sperm motility and proper capacitation, the β‐defensin core of DEFB126 has demonstrated potent lipopolysaccharide‐binding and anti‐inflammatory activities, suggesting that it may also contribute to innate immune defense within the reproductive tract (3). Epigenetic alterations in DEFB126 have further been implicated in mediating age‐related declines in fertilization efficiency (8).
