NAPSA Gene

Name napsin A aspartic peptidase
Description This gene encodes a member of the peptidase A1 family of aspartic proteases. The encoded preproprotein is proteolytically processed to generate an activation peptide and the mature protease. The activation peptides of aspartic proteinases function as inhibitors of the protease active site. These peptide segments, or pro-parts, are deemed important for correct folding, targeting, and control of the activation of aspartic proteinase zymogens. The encoded protease may play a role in the proteolytic processing of pulmonary surfactant protein B in the lung and may function in protein catabolism in the renal proximal tubules. This gene has been described as a marker for lung adenocarcinoma and renal cell carcinoma. [provided by RefSeq, Feb 2016]
Summary
{"type": "root", "children": [{"type": "p", "children": [{"type": "t", "text": "\nNapsin A is an aspartic proteinase predominantly expressed in type II pneumocytes where it performs a key role in the proteolytic processing of pro–surfactant protein B, a function essential for the generation of active pulmonary surfactant. Its expression in lung tissue and its high sensitivity as an immunohistochemical marker have established its utility in the diagnosis and subclassification of lung adenocarcinomas, particularly when used in combination with thyroid transcription factor‐1 to improve both sensitivity and specificity. This critical role in pulmonary homeostasis and cancer diagnostics was first highlighted in studies focusing on non‐small cell carcinomas, where napsin A demonstrated superior performance in identifying adenocarcinoma compared to traditional markers."}, {"type": "fg", "children": [{"type": "fg_fs", "start_ref": "1", "end_ref": "7"}]}, {"type": "t", "text": "\n"}]}, {"type": "t", "text": "\n\n"}, {"type": "p", "children": [{"type": "t", "text": "\nBeyond its pivotal role in pulmonary biology, napsin A expression extends to several non–lung neoplasms where it assists in diagnostic differentiation. It is expressed in specific subsets of renal cell carcinomas (especially papillary types), ovarian clear cell carcinomas, endometrial clear cell tumors, and certain mesothelial as well as gynecologic lesions. The distinct immunoprofile of napsin A, particularly when evaluated as part of a panel with markers such as TTF‐1, PAX8, p63, CK5/6, and hepatocyte nuclear factor‑1β, provides critical support for distinguishing primary lung adenocarcinomas from metastatic lesions and for accurately subtyping tumors with similar histomorphology. This expanded expression pattern underlines its diagnostic value across a broad spectrum of carcinomas."}, {"type": "fg", "children": [{"type": "fg_fs", "start_ref": "8", "end_ref": "22"}]}, {"type": "t", "text": "\n"}]}, {"type": "t", "text": "\n\n"}, {"type": "p", "children": [{"type": "t", "text": "\nRecent mechanistic studies have further expanded our understanding of napsin A by implicating it in tumor biology and response to therapy. Not only does its enzymatic activity contribute to the maturation of surfactant proteins in the lung, but experimental evidence also suggests that napsin A may exert tumor‐suppressive effects—including inhibition of anchorage-independent growth and attenuation of epithelial–mesenchymal transition, which in turn can enhance sensitivity to targeted therapies such as EGFR tyrosine kinase inhibitors. Moreover, alterations in its expression have been correlated with clinicopathologic parameters and patient outcomes, underscoring the biomarker’s prognostic significance. These insights, derived from a combination of molecular, immunohistochemical, and functional studies, highlight the multifaceted clinical importance of napsin A both as a diagnostic marker and as a potential modulator of tumor behavior."}, {"type": "fg", "children": [{"type": "fg_fs", "start_ref": "23", "end_ref": "47"}]}, {"type": "t", "text": "\n"}]}, {"type": "rg", "children": [{"type": "r", "ref": 1, "children": [{"type": "t", "text": "Justin A Bishop, Rajni Sharma, Peter B Illei "}, {"type": "b", "children": [{"type": "t", "text": "Napsin A and thyroid transcription factor-1 expression in carcinomas of the lung, breast, pancreas, colon, kidney, thyroid, and malignant mesothelioma."}]}, {"type": "t", "text": " "}, {"type": "i", "children": [{"type": "t", "text": "Hum Pathol (2010)"}]}, {"type": "t", "text": " DOI: "}, {"type": "a", "children": [{"type": "t", "text": "10.1016/j.humpath.2009.06.014"}], "href": "https://doi.org/10.1016/j.humpath.2009.06.014"}, {"type": "t", "text": " PMID: "}, {"type": "a", "children": [{"type": "t", "text": "19740516"}], "href": "https://pubmed.ncbi.nlm.nih.gov/19740516"}]}, {"type": "r", "ref": 2, "children": [{"type": "t", "text": "Sanjay Mukhopadhyay, Anna-Luise A Katzenstein "}, {"type": "b", "children": [{"type": "t", "text": "Subclassification of non-small cell lung carcinomas lacking morphologic differentiation on biopsy specimens: Utility of an immunohistochemical panel containing TTF-1, napsin A, p63, and CK5/6."}]}, {"type": "t", "text": " "}, {"type": "i", "children": [{"type": "t", "text": "Am J Surg Pathol (2011)"}]}, {"type": "t", "text": " DOI: "}, {"type": "a", "children": [{"type": "t", "text": "10.1097/PAS.0b013e3182036d05"}], "href": "https://doi.org/10.1097/PAS.0b013e3182036d05"}, {"type": "t", "text": " PMID: "}, {"type": "a", "children": [{"type": "t", "text": "21164283"}], "href": "https://pubmed.ncbi.nlm.nih.gov/21164283"}]}, {"type": "r", "ref": 3, "children": [{"type": "t", "text": "Bradley M Turner, Philip T Cagle, Irma M Sainz, et al.
