In the intestinal epithelium, increased NOTUM production by aged Paneth cells leads to diminished Wnt signaling, contributing to a decline in stem cell function and impaired tissue regeneration; inhibition of NOTUM can rescue this defect. In colorectal cancer models, APC‐mutant cells secrete higher levels of NOTUM to suppress Wnt signaling in neighboring wild‐type cells, thereby promoting clonal expansion and tumorigenesis. In hepatocellular carcinoma, NOTUM is upregulated as a direct target of β‐catenin/TCF, while its elevated expression in colorectal tumors and in the broader secretome has been suggested as a potential biomarker.
