Description The protein encoded by this gene is an isozyme of very long-chain acyl-CoA synthetase (VLCS). It is capable of activating very long-chain fatty-acids containing 24- and 26-carbons. It is expressed in liver and associated with endoplasmic reticulum but not with peroxisomes. Its primary role is in fatty acid elongation or complex lipid synthesis rather than in degradation. Its downregulation—whether by DNA hypermethylation or other mechanisms—results in the loss of its antiproliferative effects and contributes to tumor progression, poor prognosis, and resistance to sorafenib therapy. Mechanistically, SLC27A5 deficiency triggers alterations in lipid profiles that lead to oxidative stress through increased lipid peroxidation and the accumulation of toxic by‐products such as 4-hydroxy-2-nonenal. These modifications activate the KEAP1/NRF2 axis and upregulate downstream targets (for example, TXNRD1 and GSR) that impair ferroptosis. Its deficiency or mutation can lead to an accumulation of unconjugated bile acids—particularly cholic acid—which in turn activates hepatic stellate cells via increased expression of early growth response protein 3 (EGR3). This biochemical disturbance predisposes the liver to fibrotic remodeling.
